Autologous fat treatment

A face that is smooth and soft with harmonious transitions in youth can see its contours harden over time. If we compare the faces of adolescents and old people, a lack of structure-giving volume is particularly noticeable in addition to the age-related wrinkles.

Wrinkle treatment using natural autologous fat

With autologous fat therapy we return your lost facial volume making it appear fresher, healthier and more youthful. For this purpose, we use your own fat, previously removed from elsewhere in the body to build up volume. There is no more natural active ingredient than replacing a lack of material with the same material. Through the autologous fat injection we fill areas with lost volume again and bring back harmony to the facial transitions.

Use of the right amount is crucial here: too much volume would be displeasing and irritating. An operated sprayed-on look would be the result and should be avoided.

How does autologous fat treatment work?

In the course of the natural aging process, the skin not only loses its elasticity, but the structural fatty tissue sinks and recedes. Sometimes just the loss of volume makes a face look old and hard. In recent years, plastic surgery has come to the realization that not only a smooth surface, but just the return of volume achieves harmonization and rejuvenation. Volume can be administered for example by fillers.

Autologous fat as a natural filler

For a low-complication treatment, materials are used which occur naturally in the body, such as hyaluron. These fillers however, degrade comparatively quickly. Through modern facelift procedures, it is possible to bring the structural fat pads back to their origin and to make the skin smooth. Lost tissue however is not replaced in this way.

This is how the autologous fat treatment works (Lipotransfer)

By returning volume and thus structure through autologous fat transplantation, we can create a natural, rejuvenated appearance without the use of artificial foreign material. In the process, fat cells are first removed from another body site and processed. The fatty tissue obtained from your body is then distributed finely in the areas lacking volume. There the fat cells grow to a high percentage again and we achieve a lasting result.

What should be considered before treatment with autologous fat?

As part of your first meeting at the Nymphenburg clinic for Plastic and Aesthetic Surgery in Munich, you describe your expectations for your autologous fat remodeling. We will analyze your face in detail and present you with possibilities and recommendations. It may happen that volume alone is not enough, but a more invasive procedure, possibly with a facelift, is recommended to achieve your goals. Photographic documentation over the area is carried out.

No special preparation on your part is necessary before the operation. However, the use of aspirin two weeks prior to surgery should be avoided to minimize the risk of postoperative bleeding. The consumption of alcohol and nicotine should also be reduced to a minimum. Otherwise,  specific preparation depends on any further measures being carried out during the procedure. In the case of fat suction being perfofmed not only for the purpose of fat production but also for the correction of disturbing fat pads, some suitable preparation will be necessary.

How does the treatment go?

As a rule, autologous fat transplantation takes place on an outpatient basis under twilight anesthesia and local anesthesia at the Nymphenburg Clinic for Plastic and Aesthetic Surgery in Munich. After another short discussion and the marking, you will be taken to the operating room and the anesthesia is administered.

The fat is suctioned from a location of your choice and collected, washed and concentrated in a closed, highly sterile and air tight system. From this closed system, the fatty tissue is transferred to small syringes and distributed with fine cannulas in the areas to be treated. The fat cells will gain access to the bloodstream in the tissue and grow. At the end of the operation, only small plasters are applied to the incision areas. If irritating fat pads have been removed, a suitable compression wash is applied.

What happens after your fat treatment?

Moderate pain is expected during the first days after surgery. This can be well managed with cooling and oral painkillers. At the beginning there is clearly too much volume. This is due to a natural swelling and some overcorrection which is necessary because not all grafted adipose tissue will survive (see above). In addition, the rate of increase differs from person to person and therefore can not be reliably predicted. Clear explanations for this are not yet known.

On the day following the operation, you report to our practice again, and your bandages will be removed. Basically, we advise our patients to cool the operated areas to minimise swelling as much as possible. Also, the stress on the transplanted cells is reduced by a decreased metabolism when cooling and increases the survival rate of fat cells. You will subsequently visit our offices regularly. Sporting activities, sauna and swimming should be avoided for four weeks.

In the areas of liposuction you should wear the necessary compression underwear consistently for four to six weeks to avoid any dents. For two to three weeks you should minimise social obligations.

In the first six weeks after surgery, the swelling will go down. The treated areas will regain some volume over the weeks that follow. This is because the fat cells recover after the stress of the transplant and begin to store fat. Any hematomas will also disappear within the first two to three weeks after surgery.

Possible complications and risks

As with all procedures, complications are possible, but generally very rare. Asymmetries triggered by an asymmetrical swelling or hematoma can occur. Swelling and hematoma of the skin are normal and subside within two to three weeks. For this period it makes sense not to involve yourself in social obligations. Infections are extremely rare and can usually be treated well by the use of antibiotics.

It is quite normal that not all fat cells will grow. We assume that as a rule about 50 – 60% of the transplanted fatty tissue will increase (“take rate”). This is because not all fat cells are integrated into the tissue so that they gain access to the blood circulation. The fat cells that dont grow are absorbed by the body. In this respect, a certain amount of “loss” must always be expected.
