Eyelid lift - Nymphenburger Praxis für Plastische & Ästhetische Chirurgie

We read each others eyes like an open book: in them we recognize cheerfulness, sadness, laughter, anger and many other emotions. With age, many people complain that they look much sadder and more tired than they actually feel. The reason for this is, drooping eyelids or puffy bags often disguise emotions, and the reflection does not want to match our real feelings anymore.
An eyelid lift can help here and bring about an optimistic, vibrant appearance.

Age can also cause the eyelids to relax and to proliferation of the skin, and so-called drooping eyelids can form that can also lay a curtain in the field of view.

On the other hand, in some patients, the lower eyelid becomes more pronounced with increasing age, such as wrinkled skin excesses and bags or dark circles under the eyes.

The eyelid (Latin: Palpebra, Greek: Blepharon) is the anterior border of the eye socket. From the outside in it consists of skin, connective tissue, muscles and mucous membrane. The eyelashes are located at the edges of the eyelids, and within these the excretory ducts of the scent and sebaceous glands which lie in the eyelids. The upper and lower eyelids meet at the nose and go out towards the temple and form the inner and outer angle of the eyelid.
An imaginary straight line through the corner of the eye (canthus) defines the eyelid axis. The upper eyelid is divided in two when the eyes are open through the upper eyelid fold. There are ethnic differences in the distance of the eyelid edge. In caucasians the distance is between 8-10 mm. In Asians however, this is sometimes less than 3mm.
The eyelids protect the eyes and keep them moist. Through the blink of an eye, the tear fluid is spread over the eye and at the same time foreign bodies are transported into the inner lid angle. Tears run down the tear canal and tear ducts into the nasal cavity. The eye and eyebrow area is very well supplied with expressive muscles and plays an important role in the facial expression. During the natural aging process, the facial expression changes and the eyes begin to look tired:

Drooping eyelids

Drooping eyelids may form on the upper eyelid, as the skin above the eye loses its elasticity and excess skin occurs, causing them to lay like a curtain over the field of vision. Sometimes, in addition to excess skin, there is an increase in fat; in unfavorable cases, the visual field is restricted.

Bags and circles under the eyes

The range of problems on the lower eyelids ranges from a small amount of excess skin with wrinkling to pronounced cushion-like protrusions. The eye socket is filled by fat pads that surround the eyeball. The individual fat pads are divided by connective tissue into individual spaces. To the front, the fat chambers are also bordered and held by connective tissue fascia. Due to aging processes or congenital weakness of the connective tissue, the connective tissue structures may stretch, leading to an advance of the fat pads. The typical retractions of the skin are created by fibers that run from the bone to the skin.

An eyelid repair, the so-called blepharoplasty, aims to make your eye area more open, alert and thus younger. During the eyelid lifting we remove excess skin and fatty tissue on the upper and / or lower eyelid. The extent to which this happens depends on your individual situation.

As part of your initial consultation at our Clinic for Plastic and Aesthetic Surgery in Nymphenburg, Munich, you inform us of what your expectations from your eyelid surgery are. We will analyze your eye area in detail and work with you to develop a tailor-made upper and lower eyelid lifting plan.

Please avoid using aspirin two weeks before surgery to minimize the risk of postoperative bleeding. The consumption of alcohol and nicotine should also be reduced to a minimum.

Marking the marker points on eyelid tighteningAccording to the plan drawn up with you, the incision and important marking points of the face are drawn before administering anesthesia. As a rule, the operation is performed under local anesthesia and sedation (twilight sleep) and monitored by our experienced anesthetist. 

Modern surgical techniques are very sophisticated and take various problems into account. We therefore always make sure not to remove too much fat in both the upper eyelid and the lower eyelid, as this would create an unnatural "hollow" eye.

Modern methods are characterized by redistributing the existing fat in order to achieve a harmonious image. On the lower eyelid in particular, it must be ensured that it is stable on the eye, without puckering. Nowadays, a lower lid procedure is almost always associated with lateral suspension of the lower eyelid, which initially leads to a slightly elevated eyelid axis. This is the desired effect and returns to normal after some time.

The operation will initially cause significant swelling. This can lead to a temporarily incomplete eyelid closure during upper eyelid tightening. However, this condition is short-lived and returns to normal within a day or two. The most severe swelling disappears within days of the surgery.

Fluid retention however, can last for up to six weeks. Minimal swelling persists up to half a year after surgery. Immediately after an eyelid lift, damp, cold compresses and cooling elements are applied. This prevents increased swelling and formation of hematoma. Slight hematomas are normal and disappear within two weeks.

During this time, you should abstain from social obligations and take some rest. Also, creams and make-up should be temporarily avoided to avoid the risk of wound infection. Sunglasses help to conceal the normal side effects after eyelid lifting.

Despite the great care taken, complications can occur with every surgical procedure, however they are very rare. We rarely see rebleeding, but this is possible in the first few days following the operation. As a prophylactic measure you should therefore treat yourself to the necessary rest, so as not to provoke any rebleeding due to peaks in blood pressure. Swelling may result in a temporary lack of eyelid closure or protrusion of the eyelid from the eyeball. In older patients in particular, the risk of so-called ectropion is slightly increased due to lack of tissue tension. However through a suitable surgical method, this can be taken into account in preoperative planning. However, if the symptoms persist for longer than six weeks, a second corrective procedure should be considered.

If you are predisposed to unfavorable scarring, the scars may become hardened for a prolonged period. However, suitable scar treatments can also be successful should this occur. Of course, we will advise you in detail on the necessary measures.

A temporary dryness of the eye may occur. If you suffer from dryness of the eyes even before the correction, this symptom can be more severe. The appropriate eye drops provide relief for this.
